Initial Comments:
An offsite follow-up survey conducted on November 2, 2023, found that Transitions Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification and State licensure survey completed September 8, 2023.
